Eligibility Requirements

Job Loss	Employed for at least 90 consecutive days Employed at least 20 hours per week (10 hours per week for students enrolled in post-secondary education) Unemployed for at least 30 consecutive days	Scotia Credit Card Protection: Job Loss Form

Disability	It has been determined by Your Doctor that You are prevented by any medical condition from performing Your normal activities of daily living or the regular duties of Your employment or self-employment, as applicable, for a period of at least 30 consecutive days. Critical Illness	Must suffer from one of the covered illnesses under this plan which are defined in the Certificate of Insurance: Cancer, Heart Attack, Stroke, Coronary by-pass surgery, Coma, Blindness, Deafness, Paralysis	Scotia Credit Card Protection: Critical Illness Form
Hospitalization	Admittance to and conﬁnement in a Hospital in Canada or the continental United States (plus Alaska and Hawaii) as an In-Patient overnight for medical treatment, based on the recommendation of a Physician, due to Accidental Bodily Injury or unforeseen Sickness.
