To apply for a Scotiabank MasterCard® or Scotiabank Gold MasterCard® credit card, please download, print, and complete the Application Form and bring it to any Scotiabank branch with the following items:
- Two forms of government ID (eg. Passport, Drivers Licence)
- Utility Bill (for proof of address)
- Job letter or pay slip
