Steps to Download an Application

1. Save the application form [PDF: 64 kb] to your hard drive. Remember to make a note of the filename.

2. Open the PDF file on your hard drive. This will automatically launch Acrobat Reader.

3. In Acrobat Reader select, File, Print from the toolbar and print the application.
