Credit Card Application Form

Please download, print, and complete the Credit Card Application Form and bring it to any Scotiabank branch with the following items:
  • Two forms of government ID (eg. Passport, Drivers Licence)
  • Utility Bill (for proof of address)
  • Job letter or pay slip
